How to Report a Complaint to the Office of the State Inspector General
There are several ways to report fraud, waste, abuse or corruption to the OIG. Complaint forms may be submitted online or downloaded from our web site and then mailed or faxed.

Confidentiality:
The OIG will not, after receipt of a complaint from a public employee, disclose the identity of the employee without consent, unless the OIG determines that such disclosure is unavoidable during the course of an investigation.
OIG is not obligated to keep complaints received from non-public employees confidential or to maintain the anonymity of non-public employees. However, non-public employees may specifically request confidentiality, and the OIG will protect the confidentiality to the extent possible during investigation.
Protection from Retaliation:
State law provides penalties for public employers who retaliate against public employees who report misconduct to the OIG. If you experience retaliation, you may report the incident to the Office of the Inspector General.
